Referrals

Instructions for Referring a Patient:

-Please download and complete the referral form.

-Fax a copy of the form to 800-292-4120.

-Please include any additional medical history information/files.

-Our office staff will process the referral and contact the parent within 2-3 business days.

-Please include a physician prescription that states “speech and/or occupational therapy to evaluate and treat”.

-Prescriptions are valid for six months of therapy services.

Current in-network insurances accepted include: Tricare East, Georgia Medicaid, Peach State Health Plan, Amerigroup, and Ambetter. Some out of network insurance maybe accepted pending review. Discounted cash pay services are also available. Please inquire via email.

Thank you for referring a patient to Discovery Therapy Network! Our organization is committed to supporting the therapeutic needs of babies, toddlers, and children.  We provide speech therapy and occupational therapy services to help children with developmental delays from birth to age 21.  Discovery Therapy Network specializes in teletherapy services, family education webinars, parent support groups, therapy coaching and professional consultations.  Our therapists provided extensive evaluations and are trained with skills to contribute to Individualized Family Service Plans (IFSP) or Individual Education Plans (IEP) to enhance functional skills.  

Coaching services are available to families to guide the incremental progress of advancements for speech, communications, language, sensory processing, fine motor, activities of daily living, and visual perceptual skills.  Our therapy professionals will work with service coordinators, social workers, medical practitioners, child development specialist and other professional specialists to ensure achievement of growth and development. 

Our teletherapy services use HIPAA compliant connectivity to protect patient privacy.  Our skill building approach increases communication, fine motor, gross motor, and sensory self-regulation skills.  Currently our services are being offered in several counties throughout the state of Georgia via Teletherapy.
